Gynecology Department is the level-1 key medical discipline of Hangzhou and a department under the unified management and deployment of Hangzhou First People’s Hospital Group.

Academic leader: President ZHANG Zhifen. The department has 6 chief physicians and 9 associate chief physicians, including 3 professors and associate professors, 3 doctors and 2 research supervisors as well as 2 experts, and provides services in connection with general gynecology, gynecological oncology, pelvic floor, family planning, reproductive endocrinology, etc. As for clinic services, there are clinics for gynecology, gynecological oncology, cervical disease, pelvic floor, perimenopause, reproductive endocrinology (infertility), family planning, etc. 208 beds are proposed and 84 opened. The department boasts advanced equipment, and multiple imported high-definition electronic hysteroscope, laparoscope, colposcope, etc. and regularly invites top experts in China including Prof. LANG Jinghe of Peking Union Medical College Hospital and Prof. SHI Yifu of Women's Hospital School of Medicine Zhejiang University to guide and participate in medical treatment. It has undertaken many national, provincial and internationally-collaborated research projects, some of which have won provincial award for progress in science and technology, and has organized classes for national, provincial and municipal continuing medical education. It is a national base for continuing education in gynecological endocrinology.

1. Gynecologic laparoscopy: The department has carried out such surgeries as laparoscopic hysterectomy, myomectomy, oophorocystectomy, adnexectomy, conservative or radical surgeries for endometriosis and adenomyosis, conservative or radical surgeries for ectopic pregnancy, and laparoscopic repair for pelvic floor dysfunction. More than 85% of the surgeries are performed with laparoscope, which is also applied to surgeries for gynecologic malignancies, including extensive hysterectomy and pelvic lymphadenectomy. Hysteroscopic uterine fibroids, endometrial ablation, transcervical resection of adhesions, metroplasty for uterine malformation, hysteroscopy-laparoscopic treatment of infertility and complicated family planning surgery, laparoscopic corrective surgeries for uterine and vaginal prolapse can also be performed here.

2. All kinds of vaginal surgeries: Such as transvaginal hysterectomy, subtotal hysterectomy, myomectomy, vaginal reconstruction, surgical treatment of urinary fistula and fecal fistula, as well as scarred uterus diverticulectomy.

3. Diagnosis and treatment of pelvic floor diseases: Corrective procedures for metroptosis, anterior and posterior vaginal wall prolapse, stress incontinence and congenital reproductive malformation, conservative electrophysiological therapy for metroptosis and vaginocele caused by stress urinary incontinence and pelvic floor defects, conservative therapies for complicated pelvic and vaginal pain and sexual dissatisfaction, and surgical treatment of sexual dissatisfaction: vaginal tightening surgery and mini-plastic vaginal surgery for vaginal relaxation. Vaginal sacrospinous ligament suspension is currently a relatively advanced surgical procedure in the world.

4. Reproductive endocrinology: With a strong technical strength, the Reproductive Endocrinology Center is capable, by leveraging its well-equipped laboratory and strong technical force, for examination, diagnosis and treatment of infertility caused by various reasons and standard diagnosis and treatment of polycystic ovary syndrome, hyperpro-lactinemian and amenorrhea. It sets up the Infertility Clinic for ovulation induction and ovulation monitoring, various types of surgical treatment of female infertility including hysteroscopic, laparoscopic and other minimally invasive assisted reproductive surgery, tubal adhesion release, re-canalization and plastic surgery combined with hysteroscopy and laparoscopy for treatment of infertility, etc. Hormone replacement therapy after frozen embryo transplantation, preventive treatment of various threatened abortion, elective/emergency cervical cerclage for cervical incompetence, and standard treatment of ovarian hyper-stimulation syndrome (OHSS) are also performed here.

5. Family planning: Standardize the implementation of painless visual abortion and diagnosis and timely treatment of various complicated termination of pregnancy and heterotopic pregnancy. Standard treatment of early cesarean scar pregnancy: based on classified assessment on gestational sac adhesion, such methods as injection plus uterine curettage, uterine curettage or surgical resection after uterine artery interventional embolization are used to reduce the risk and preserve the uterus.

6. Treatment of stubborn and acute gynecological diseases: relying on the strong comprehensive rescue ability of the Hangzhou First People’s Hospital Group, it is able to diagnose and treat all kinds of complicated and stubborn gynecological diseases.

7. Diagnosis and treatment of cervical diseases: The hospital possesses a variety of cervical disease screening techniques and provides internationally recognized HPV DNA test, HPV typing test and high-risk HPV typing test and strict cytologic examination such as TCT and imported high-definition colposcopy to make a hierarchical screening and management of cervical diseases and relevant standard hierarchical treatment and follow-up visit. There is a well-equipped outpatient operating room, where colposcopy and cervical LEEP for cervical diseases and intraepithelial neoplasia can be completed.

8. Diagnosis and treatment of gynecological tumors and common diseases: Comprehensive implement comprehensive therapy and individualized therapy such as radical operation, systemic chemotherapy and other biological treatments for gynecological malignancies including cervical cancer, endometrial cancer, ovarian cancer, fallopian tube carcinoma and carcinoma of vulva; have extensive experience in comprehensive treatment of pelvic inflammatory diseases and stronger ability to combine Chinese traditional medicine and western medicine; have unique surgical and integrated therapy of traditional Chinese medicine and western medicine for endometriosis.

9. Senile gynecology: Home to the Perimenopausal Diagnosis & Treatment Center, it has rich clinical experience in prevention and treatment of common diseases for menopause ad senile women such as dysfunctional uterine bleeding, perimenopausal syndrome, osteoporosis, endometriosis, senile pelvic floor dysfunction and gynecological oncology, and advocates hormone replacement therapy for perimenopausal women.

10. Education of disease prevention: Regularly hold health lectures for patients to improve the patients’ awareness and self-care level and guide reasonable lifestyles, and provide relevant counseling services.

11. I. German and domestic experts personally guide all kinds of surgeries for pelvic floor dysfunction.

1. Surgeries for treatment of metroptosis with uterus preserved: Anterior sacrospinous vaginal vault suspension, with ideal effect, small trauma and few complications, is one of the domestically leading and internationally advanced surgeries. After follow-up visit to patients having undergone the surgery in our hospital, it is found out that the results are satisfactory and the complications are less than other surgical treatment.

2. Various surgeries for treatment of uracratia such as TVT, TVT-O and TOT.

II. Hysteroscopic minimally invasive surgery; Hysteroscopic shaving system makes the endometrium free of electro-thermal damage during surgery, which is especially suitable for uterine submucous leiomyoma with fertility demand. It is an advanced surgical treatment in China.

III. Relying on the strength of the Hangzhou First People’s Hospital Group, the hospital boasts the most complete and advanced hardware facilities (such as Imaging Center, Radiotherapy Center, Endoscopy Center, Biotherapy Center and Hyperthermia Center) for gynecological malignancy diagnosis and treatment in the province and a provincially renowned team in gynecological malignancy diagnosis and treatment, having its own special characteristics for standard diagnosis and treatment of gynecological malignancy.

The characteristics of four common types of gynecological oncology are as follows:

1. Gestational trophoblastic tumors: The department’s comprehensive treatment of gestational trophoblastic tumors is at the leading level in Zhejiang province. It receives patients suffering from drug-resistant and recurrent gestational trophoblastic tumors from the province and even the country, and has received and cured patients suffering from gastrointestinal, hepatic and brain metastasis of gestational trophoblastic tumors and some other high-risk and refractory gestational trophoblastic tumors. With combined treatment of individualized stratified therapy, surgery, chemotherapy and radiotherapy, patients with rare placental site trophoblastic tumor and epithelioid trophoblastic tumor have also been accepted and cured here. Our diagnosis and treatment of hydatid mole - premature lesion of gestational trophoblastic tumors is at the leading level in China. Besides, it has 2 research projects supported by National Natural Science Foundation of China, 1 supported by major special fund of Science Technology Department of Zhejiang Province, and Zhejiang Natural Science Foundation, totaling over two million yuan. Moreover, it has carried out cooperated research with scholars from McGill University that is at the leading position in research on hydatid mole in the world, having made some achievements in the field of familial recurrent moles in Chinese families and etiology and molecular genetics research of recurrent moles, which may guide relevant clinical practice.

2. Ovarian cancer: The department’s surgical procedure for advanced ovarian cancer and recurrent ovarian cancer is at the provincially advanced level. For transverse pelvic diaphragm and other peritoneal lesions, the megascopic tumor lesions of diffuse metastasis are excised as far as possible in a carpet way. Cytoreductive surgery of ovarian malignant tumor has a high idea reduction rate (residual lesions less than 0.5cm). With advanced comprehensive treatment of ovarian cancer in the province, it also applies intra-operative thermo-perfusion chemotherapy and postoperative tumor gene detection for individualized selection of sensitive chemotherapeutics. The department is home to provincially advanced biotherapy center and hyperthermia center as well as good support of sterile laminar flow ward and intensive care unit for patients with severe bone marrow suppression after chemotherapy of malignant tumor, guaranteeing the safe completion of therapeutic schedules.

3. Standard diagnosis and treatment of cervical cancer: It has applied effective screening approaches and given standard treatment such as TCT, HPV and colposcopy for precursor lesions of cervical cancer; LEEP and CKC for cervical intraepithelial neoplasias; preoperative interventional neoadjuvant chemotherapy for gynecologic malignant tumor of patients with stage Ib2 and IIa2 massive cervical cancer; laparoscopic radical resection of cervical cancer, ovarian sparing surgery for cervical cancer, reproductive organ sparing surgery for cervical cancer and vaginal lengthening, in addition to regular abdominal radical hysterectomy, for stage Ia2-IIA1 cervical cancer to improve the living quality of patients; for patients with high risk factors after early-stage cervical cancer surgery and those suffering from medium-term and advanced cervical cancer and recurrent cervical cancer, we, equipped with a Gynecological Radiotherapy Center at the leading level in the province, can provide comprehensive treatment.

4. Endometrial cancer: It has carried out precursor lesion of endometrial cancer screening management; has certain foundation in diagnosis and treatment of endometrial hyperplasia and endometrial cancer with reproductive organ preserved; has carried out standard diagnosis and treatment of endometrial cancer and conducted preoperative comprehensive evaluation based on pathology of endometrial cancer; for the patents with operation indications, it has performed pelvic and abdominal aortic lymphadenectomy with extremely low incidence rate of intra-operative complications; for the patients with high risk factors after operation and recurrent endometrial cancer, combined treatment of chemoradiotherapy has been adopted.

In addition, the department carries out diagnosis and treatment of precursor lesion of vulva and radical vulvectomy as well as relevant combined treatment of Chinese traditional medicine and western medicine.
